Support at Home Care Coordination
Supporting older Australians across the Illawarra, Shoalhaven, Southern Highlands and surrounding NSW areas.
Local support. Personal coordination. More control over your care.
Navigating aged care funding, services, budgets and providers can feel complicated. It doesn’t have to be.
Nursing Solutions Group provides local Care Coordination for Support at Home participants, working in partnership with Trilogy Care, a registered Support at Home provider.
Our role is simple: we help you understand your funding, organise the services you need and remain your local point of contact as your needs change.
You don’t have to spend your time calling different providers, trying to understand your budget or wondering who you need to speak to. Our team works alongside you and your family to coordinate your care and help make your Support at Home funding work for you.
Care based on what matters to you
Support at Home is designed to help older Australians remain independent, safe and connected while continuing to live at home.
Every person is different.
Some people may only require a little assistance around the home. Others may need personal care, nursing, transport, allied health, equipment or increasing support as their health changes.
That is why our approach starts with you!
We take the time to understand:
- what is important to you
- what you can continue doing independently
- where you would like some assistance
- your health and care needs
- your routines and preferences
- your family and existing support network
- the services you are already using
- your goals for remaining at home
- how you would like your Support at Home funding to be used
From there, we help coordinate the people and services around you.
What Our Care Coordination Includes
From coordinating everyday services to responding when your needs change, our team is here to help keep your care organised, connected and centred around you.
Care coordination
We help organise your services, appointments and supports and make sure everyone involved understands what is required.
Regular contact and review
Your circumstances can change. We stay in contact with you and review whether your current services continue to meet your needs.
Support workers and personal care
We can coordinate suitable workers to assist with personal care, domestic assistance, social support, transport, respite and other approved services.
Nursing and clinical support
As a nursing-led organisation/associate, we can help identify when clinical input may be required and coordinate nursing or other health professionals where this is included within your approved supports.
Budget awareness
We work with you and Trilogy Care so you understand your available funding and can make informed decisions about how it is used.
Changes in care needs
If your health or circumstances change, we can help review your services and assist you through the process of seeking additional assessment or support where appropriate

Partnering with Trilogy Care
Trilogy Care is the registered Support at Home provider within our partnership.
They work with Nursing Solutions Group and you to manage the formal requirements associated with your Support at Home funding, including your care plan, individualised budget, funding administration, claims, statements and provider governance requirements.
Together, this means you receive the benefits of a registered Support at Home provider while still having a local Nursing Solutions Group team who knows you and coordinates your day-to-day care.
How It Works
You remain involved in every decision
Support at Home is built around your rights, preferences, independence and choice. Being supported does not mean giving up control.
We explain your options so that you can make informed decisions about your own care. You can involve family, a registered supporter or another person you trust in discussions about your care if you choose. Your care plan and services should reflect what matters to you and change when your needs change.
1 Meet with us
We start with a conversation.
We want to understand your current situation, your Support at Home funding, your goals and what is most important to you.
If family members or a registered supporter are involved in your care, they can be part of these conversations too.
2 Set up your Support at Home services
We work with Trilogy Care to establish your Support at Home arrangements.
Your assessed needs, goals and approved services are used to develop your care plan and individualised budget.
We will explain what this means in practical terms — including what services you can access and how much funding is available.
3 Build your care around your life
Together, we work out what services you need and when you need them.
This may include:
- Personal care and daily living assistance
- Domestic assistance and household support
- Social support and community participation
- Transport and appointments
- Respite and support for carers
- Nursing services
Allied health services - Meal preparation and nutrition support
- Home maintenance and gardening
- Assistive technology
- Home modifications
- Continence and clinical supports
- Other approved Support at Home services
Assistive technology and home modifications are funded separately. We’ll help gather what’s needed, such as OT recommendations and quotes, and Trilogy Care applies for the funding. Nothing is purchased until it’s approved.
Services must be consistent with your assessment, Support at Home care plan and the Australian Government Support at Home service list.
4 We coordinate the services
Once your services are agreed, Nursing Solutions Group helps coordinate them.
Rather than leaving you to manage multiple organisations yourself, your Care Coordinator can assist with service arrangements and communication between the people involved in your care.
Where Nursing Solutions Group provides the service directly, we coordinate our team.
Where another provider is required, we can assist with coordinating that service within your approved care arrangements.
5 We keep reviewing your care
Care should never be “set and forget.”
Your Care Coordinator stays connected with you and monitors how your services are working.
We look at things such as:
- Are your current supports still suitable?
- Are you achieving the goals in your care plan?
- Has your health or mobility changed?
- Are there services you no longer need?
- Is something new required?
- Is your budget being used appropriately?
- Do we need to plan ahead for future support?
If something changes, we work with you and Trilogy Care to review your care arrangements.
A local team who actually knows you
One of the biggest differences with Nursing Solutions Group is that we are not simply administering a funding package from somewhere else. We are a local care and nursing organisation.
Our team works across our communities supporting older people in their homes every day. That means when you call us, you are speaking with people who understand local services, local health networks and the practical realities of organising care at home.
As your needs change, you have someone you can contact who already understands your circumstances.

Can you help me understand my My Aged Care assessment?
Yes.
Aged care terminology and assessments can be confusing.
We can go through your assessment, approved services and funding with you and explain what they mean for the care you can receive at home.
What if I don’t have Support at Home funding yet?
You can still contact us.
We can explain the general process and help you understand where to start with My Aged Care.
Eligibility and funding decisions are made through the Australian Government aged care assessment process.
Do you have a clinical team?
Yes.
Nursing Solutions Group is a nursing-led organisation with an experienced clinical team supporting our Care Coordination and frontline workforce.
Our clinical team works alongside Trilogy Care’s clinical team to support nursing assessments, changes in health needs and complex care. They can also assist with medication-related concerns, wound care, continence, chronic health conditions and other clinical supports where appropriate.
Having access to experienced clinical teams means changes in a client’s health can be identified and escalated early, rather than simply being treated as a rostering issue.
Who will be my main point of contact?
Your Nursing Solutions Group Care Coordinator will be your main local point of contact for your day-to-day care and services.
We work closely with you, your family or registered supporter and Trilogy Care to help coordinate your services, monitor your care and respond when your circumstances change.
Our aim is to make your care easier to manage by giving you a local team who understands your needs.
Can Nursing Solutions Group provide my carers and nurses?
Yes.
Nursing Solutions Group has its own workforce of support workers, personal care workers, assistants in nursing, enrolled nurses and registered nurses.
Where appropriate, we can coordinate your regular care team and work towards providing consistency in the people supporting you at home.
Can you organise services that Nursing Solutions Group does not provide directly?
Yes.
Your Support at Home care may involve a range of services and professionals.
Where an approved service is not provided directly by Nursing Solutions Group, your Care Coordinator can assist with arranging suitable external providers, such as allied health professionals, equipment suppliers, home maintenance services or other specialist supports.
This allows your care to be coordinated through one central point of contact.
Can my family or another person be involved in my care?
Yes.
With your consent, your family, carer, registered supporter or another person you trust can be involved in discussions about your care, services and budget.
We encourage open communication between everyone involved while making sure that you remain at the centre of decisions about your care.
Can Nursing Solutions Group provide my support workers?
Yes.
Nursing Solutions Group has an established workforce providing aged care, home support, personal care and nursing services.
Where appropriate, we can coordinate Nursing Solutions Group team members to provide your regular services.
Can I keep an existing worker or service provider?
Potentially, yes.
Support at Home allows participants to have greater involvement in how their services are arranged, including the use of third-party workers and providers in appropriate circumstances.
Any worker or provider must meet the necessary Support at Home requirements and the service must fit within your approved care arrangements.
Speak with us before changing a service and we can check what arrangements can be made.
What is the role of Trilogy Care?
Trilogy Care is the registered Support at Home provider within our partnership.
They manage the formal Support at Home funding and provider responsibilities, while Nursing Solutions Group provides local Care Coordination and can organise and deliver your day-to-day services.
The two organisations work together so you receive both the governance of a registered Support at Home provider and the personal support of a local care team.
Do I have a say in how my funding is used?
Yes.
Your funding needs to be used for services that are eligible under the Support at Home program and consistent with your assessed needs and care plan, but you should be actively involved in deciding how your care is organised.
We will explain the available options so you can make informed decisions.
What is the Care Management funding under Support at Home?
Under the Support at Home program, 10% of each participant’s quarterly ongoing Support at Home budget is allocated by the Australian Government to care management.
Care management applies to all Support at Home participants, including people who choose to self-manage parts of their care.
It funds activities such as care planning, service coordination, monitoring, reviewing risks and goals, and helping participants understand and make decisions about their care.
Will I have regular contact with my Care Coordinator?
Yes.
Support at Home requires ongoing care management, including direct contact with participants or their registered supporter at least monthly.
More importantly, we want you to contact us when something changes rather than waiting until your next formal review.
What happens if my health or care needs change?
Contact your Care Coordinator.
We can review what has changed, speak with our clinical team where required and determine whether your existing services need to be adjusted.
If your needs have increased beyond your current Support at Home funding or assessment, we can also work with you and Trilogy Care to identify what further review or reassessment may be required.
What if I am discharged from hospital and suddenly need more help?
Let us know as early as possible.
Our team can work with you, your family, hospital discharge planners and relevant health professionals to help coordinate appropriate supports within your available funding and care plan.
Where your needs have substantially changed, we can also discuss whether further assessment may be required.
Can I change providers later?
Yes.
You have the right to make decisions about who provides your aged care.
If you are considering changing providers, we recommend discussing the transition arrangements so that your services can continue safely and there is a clear handover of your care.
